III: PROBLEM PRESENTED

The client needs medical help for his physical health condition.

IV: HISTORY OF THE PROBLEM

Daryl’s family belongs to the poverty level. Their family maintains a basic living
standards even though the income of his parents is not enough for the family needs.
According to the client Daryl after he studied grade 10 he decided to stop his studies and
he find a job.

Monday on August 31, 2020, he started working as a house boy in General Santos
city, but he didn’t think that apart from being a house boy he was also made to work on
his employers farm. Wednesday on January 20, 2021, he was ordered by his boss to
climb the mango tree to prune its branches and make it charcoal. But unfortunately Daryl
fall down from the mango tree he climbed and his back was hit on the ground and he got
injured, he was immediately taken by his boss to the manghihilot and he was not even
taken into the nearest hospital.

After taking Daryl to manghihilot his boss took him back to his family. A week later
Daryl situation suddenly worsened, he could no longer move his body, his wound became
irritated and he had difficulty in breathing. Tuesday on February 02, 2021, his parents
took him to the hospital for treatment, After a week Daryl was released from the hospital.
But when he was at home he did not follow the doctor’s advice to continue taking his
medicine for his continued recovery because his parents did not have money to buy his
medicine and and only a month later Daryl’s wound got bigger and his flesh gradually
rooted. Daryl's family did not bring him to the hospital due to financial incapability.

V: CLIENT BACKGROUND

CLIENT

The client was born on December 7, 2002, in Sitio Batotitik, Brgy Basag, Tiboli,
South Cotabato, he was fourth among the seven siblings. Before the tragedy of Daryl’s
life, he was a physically strong and fit, he loves playing basketball and horse riding. At
young age Daryl learned on how to earnned money and he started to work as a corn
harvester and laborer to support his needs and to help his family to lessen their expenses
from their daily needs. As stated by his mother Amalya her son Daryl is a very responsible
young man to their family.

FAMILY

Daryl has been living with his family for almost 18 years in Sitio Batotik, Tiboli, South
Cotabato. Daryl has 6 siblings namely Jemboy, Mae, Kibo, Stanly, Justine, and Kyle. His father
Rosa is a corn farmer and earns up To 4,000.00 pesos a month. His mother Amalya is a house
keeper and a laundrywoman at the same time and earning an average income of 3,500.00 pesos
a month. Daryl’s 2 siblings Jemboy and Mae are married and they are living separately from him
and with no sufficient income. His brother kibo is a church pastor in bukidnon, Stanly is an
incoming senior high school student this year, Justine is an incoming grade 7 student this year
and Kyle is an incoming grade 5 student this year and they are all currently enrolled in Batotik
Mission Academy Inc., and they are studying through modular class.

The family has healthy relationship and they are a God fearing people, even they are poor
as long as they were together. The family total income is only 7,500.00 pesos a month and not
enough for the family basic needs because not all the family members of the family are employed.
Their current economic status is under the poverty level D with an income per capita per day of
10.8 and the family spending food for themselves in an amount of 5,400.00 pesos and series of
electricity bills amounting Php. 200.00 pesos a month

COMMUNITY

The client community has a peaceful environment. They are practicing proper
hygiene and proper garbage disposal. The client’s house is made up of light materials
and it is near from the church and school. The client family has a positive attitude towards
their neighbors and they also actively participate in church and community activities.

Most of the populace belongs to the poor families. They have the same house
structure which made up of light materials. However electricity and water supply is
accesible. The main source of living is corn farming. Some of them are laborer,corn
harvester and bamboo stick maker.
VI: ASSESSMENT TAKEN

Daryl needs medical help for his physical condition, he has a big sore on his back
that is very serious and become infected if he is not taken to the hospital immediately for
treatment.

Daryl and his family showed a great motivation and interest to participate and
cooperate in the referral for his hospitalization.

The solution for Daryl’s problem is quiet possible because the resources and
services that could help them are available in nearby hospital and agencies can be tapped
since he is qualified to avail services like availing indigent Phil Health.

If Daryl’s problem will not addressed immediately the client physical and health
condition will be worsen and his big sore will get infected and lead him into death. I hereby
recommended him to be admitted in South Cotabato Provincial Hospital for possible
availment of his medical treatment.

VII: INTERVENTION PLAN

Goal: To facilitate the client to avail medical treatment and other services he needed
with the time frame from April 12 to May 15, 2021

IX: EVALUATION

With the client's participation and willingness to give the necessary information
during the the interview,first phase of helping process was materialized

After the initial phase, the social work intern and the client formulated the goals,
focusing on helping relationships.

X: RECOMMENDATION

Daryl was recommended to monitored by his mother Amalya until he will fully
recovered. He was also recommended to visit a spine doctor specialist.

XI: TERMINATION

In the case of the client problem presented is being terminated because the goal
has been achieve and the services has been rendered therefore the case is terminated
