Conclusion

The accident covered in this case study was an accident caused by American Airlines, and the
aircraft that crashed was a DC 10. At the company, the USAF policy was to maintain the speed
up to V2+10 when the engine failed. Pilots are not responsible in this tragedy since they were
completely focused and following the procedures laid down by American airlines. This incident
occurred as a result of an American Airlines maintenance engineering error in which
maintenance engineers used a fork lifter to assemble the engine and pylon as a single part. This
idea had previously been followed by United Airlines, but American Airlines failed to execute it
correctly. During the process even they have received the warning but they disobeyed it The
maintenance teams of American airlines didn’t inform the quality control department about the
problems and difficulties which they have faced during maintenance and the reporting systems of
were failed to find any use of improper maintenance procedure which becomes the huge cause of
accident and because of this entire system become disconnect and pilots didn’t received any
warning. when the aircraft takeoff in high speed pilot faced losing controls of flights and it starts
stalling then the pylon get broke and fell on the runway and this whole incident revolves around
the maintenance failure by airline.
