I am writing on behalf of Mrs Elizabeth Dawson to request a home based physiotherapist for her
husband, Mr Dawson, who has difficulties moving around due to a fall. He requires further
assessment and physical therapy.
On 18/05/2018, Mr Dawson fell down the stairs which has caused a severe left knee abrasion.
Therefore, his doctor has advised our facility to provide regular wound management and assistance
with showering. Although he has experienced usual pain, he has improved well with no signs of
wound infection. Besides, he can move slowly with the aid of his wife. After several days of
treatment, his knee has healed.
Based on medical history, Mr Dawson has suffered stroke 4 years ago. He had a successful
rehabilitation and is mentally alert. However some speech problems and residual left side weakness
were noted. Additionally, he walks with a limp, for which he has some impaired balance.
In view of the above, it would be greatly appreciated if you could schedule a home visit to Mr
Dawson for a thorough evaluation and provide rehabilitation therapy to improve his mobility. In
addition, he owns a walking stick, but has been ineffective because he was not taught how to use it
properly as stated by his wife.

man, who is under our care
with the complaints of a grazed left knee and probl
ems with mobility.
Mr. Dawson has a history of cerebrovascular acciden
t which occurred in
2004. He has had problems with the knee for the las
t week following an
accident when he fell down the stairs.
As per general practitioner’s order, we are doing d
aily home visits and
wound dressing and also assisting him with his show
ers. His wound is healing
and is free from infection. He walks slowly with th
e help of his wife.
Mr. Dawson, who lives with his wife in his own home
, is an aged care
pensioner. The doctor has advised him to use a walk
ing stick in order to
improve his mobility but he is unskilled in using i
t. It would be greatly
appreciated if you could arrange home visits to pro
vide training and
assistance to Mr. Dawson
