Good day. I am ZESREAL CAIN V. BANTILAN, a bonafide resident of Oroquieta City. I am

currently residing at Tibanga, Iligan City due to my currently running review for the Board Examination
for Professional Chemical Engineering.

I am currently processing for the requirements for the incentive given by from the local
Ordinance No. 839-2019, An Ordinance Granting Cash Incentives with Certificates of Recognition as
Rewards to Oroquietanian Topnotchers of Bar Examinations, PRC Board Examinations and
Tertiary/College Graduates with Latin Honors, Appropriating Funds Therefor and for such Other
Purposes. One of the requirements is the Voter’s Certification.

Due to my circumstances, I am out of town to process this document and on my behalf, I authorize my
mother Mila V. Bantilan to receive the Voter’s Certification.
