AFFIDAVIT OF OWNERSHIP

I, FAITH JOY V. BAYANI, of legal age, single, Filipino and a resident of

Brgy. Lapuz Norte, Iloilo City, after having been sworn to in accordance with law
depose and say:

That I am the owner of a one (1) yg Religious Ring with approximate

weight of 5.0 g 18k and one (1) yg Ladies Ring with Stones with approximate 4.6
g 18k with deformities;

That said one (1) yg Religious Ring with approximate weight of 5.0 g 18k
and one (1) yg Ladies Ring with Stones with approximate 4.6 g 18k with
deformities was pawned in Palawan Pawnshop, Lapuz Branch, Jalandoni St.,
Iloilo City last November 5, 2019 with Pawn ticket No. 34473 in the amount of
PhP16, 200.00; and

That I am executing this affidavit of ownership in order to attest to the truth

of the foregoing instrument.
