AFFIDAVIT OF DESISTANCE

We, GERALYN G. GUEVARRA, JINKY G. GEAGA, LOVELEEN G.

PEÑAFLOR of legal age, single, Filipino citizens with residence at Guimbal,
Iloilo, after being duly sworn to in accordance with law, depose and state that:

1. We are the private complainants in a vehicular incident which resulted to

physical injuries involving a tricycle driven by RONALDO GENEROSA;

2. That we know that it was purely an accident and RONALDO GENEROSA has
personally helped us in the hospital and medical expenses as well as extended
financial assistance in relation to the injuries we have sustained by reason of the
above-mentioned accident;

3. That we are only asking for financial assistance to the injuries we sustainded
which was primarily caused only by the the said accident;

4. That we have no intention nor an interest to pursue any complaint now or in

the future either criminal and civil case against RONALDO GENEROSA;

5. That we are executing this affidavit of desistance freely and voluntarily, without
any element of fear, violence, coercion, intimidation and whatsoever that may
vitiate our consents; and to attest to the truth of the foregoing, and for whatever
legal purpose this may serve.
