By providing my personal information in this form, I acknowledge and consent to my

personal information being collected, recorded, held, stored, used and disclosed by
SAN MIGUEL CORPORATION, its subsidiaries and affiliates, as appropriate to
evaluate my application for employment, including but not limited to conducting
reference checks and interviews. With regards to personal information about someone
other than myself, I confirm and acknowledge that I have obtained their consent for
disclosure of their personal information to SAN MIGUEL CORPORATION, its
subsidiaries and affiliates.

I understand and agree that all the information supplied by me in this form shall be
managed by SAN MIGUEL CORPORATION in accordance with the Data Privacy Act
2012, SMC Privacy Policy, and SMC policy on records retention and disposal.

I also agree to release SAN MIGUEL CORPORATION and its directors, officers,
employees, shareholders, subsidiaries, affiliates and related companies, and agents
from every and all liability (including negligence) whether direct or indirect, special or
consequential arising out of the collection, recording, holding, storage, use and
disclosure of such personal information in connection with my application for
employment.

I hereby certify that all the information supplied by me in this application form is true and
correct. I authorize SAN MIGUEL CORPORATION, its subsidiaries and affiliates,
and/or its authorized agencies to conduct reference checks. Should untruths be
discovered during my employment in any position in this Company, I fully agree that the
same is just a cause for my termination.
