Prshe na GuelneonCOVID- Seated once Dp EdDenaeCtyDIson mea Republic ofthe Pilipines Department of Education REGION XI SCHOOLS DIVISION OF DAVAO CITY DIVISION ADVISORY(No.0.0 0 4's. 2021 Guidelines on COVID-19 related Concerns in DepEd Davao City Division TO : All Teaching and Non-Teaching Personnel this Division DATE 14 JUN Zuzi In response to the surge of the COVID-19 Confirmed Cases in Davao City, and in compliance to the Executive Orders and Guidelines released by the City Government of Davao, this office reiterates Division Memorandum No. 1142 s. 2021 - Management of Covid-19 Positive, Probable, Suspect and Close Contact Cases in Schools and Workplace and issues this “Guidelines on COVID-19 related Concern in DepEd Davao City” attached as Annex A. 2. It is further reiterated that all personnel should report to their respective Section Chiefs/Heads, School Head/Prineipal and Division/DistrictCluster Nurse as soon as they know that they are affected with such. None reporting will be grounds for none issuance of Work from Home Medical Certificate or Medical Clearance issued by the DepEd, Davao City, School Health Section, 3. Upon Failure to report immediately any incident related to COVID-19 pandemic situation, shall be dealt with accordingly 4. Relevant to this, in pursuant to RA 11332 or also known as the “The 2020 Revised Implementing Rules and Regulations of Republic Act No, 11332, or the Mandatory Reporting of Notifiable Diseases and Health Events of Public Health Concern Act” specifically Rule XI on Prohibited Acts and Policies, Section 1, h, i and ii and Section 3 which states that.. “h,) Non-cooperation of the person or entities affected by a notifiable disease or a health event of | public health concern, which can be any of the following acts: i, Failure by close contacts to cooperate/submit to public health authorities doing contact tracing activities upon being notified of their status as such; ii, Violation of community quarantine or stay-at-home order or directive issued by public health authorities. Section 3, Penalties. - Any person or entity found to have committed any of the prohibited acts referred to in Section | of this Rule shall be penalized with a fine of not less than Twenty Thousand esos (P20, 000.00) but not more than Fifty Thousand Pesos (P50, 000.00) or imprisonment of not less than one (1) month but not more than six (6) months, or both such fine and imprisonment at the discretion of the proper court.” 5. The 1* (F1) and 2" (F2) generation close contacts will complete the 14 days Home Quarantine, regardless of NEGATIVE result or swabbed or not swabbed. Due to different issues on direct reporting of the F2 and F3 to their respective Barangays, we will still follow our protocol that the FI will directly report to their respective BHU. We will wait for further recommendations/instructions of the Barangays regarding the F2 and F3. While waiting: © The F2 will be on WFH/HQ for 14 days, as additional preventive measures of this agency; and © The F3s will resume work onsite but with strict compliance to health protocols, until they are informed that they are subject for swabbing. SafeDavao QR Those who were texted: ° They must immediately inform their School Head/Schoo! Principal, Distric/Cluster Nurses and Clinic-in-Charge. They are advised to start Work from Home (WFH) arrangement from the time they were texted and to call and coordinate immediately with their respective Barangay Health Centers as per residence or the SafeDavao QR Hotline for further instructions. Once swabbed, personnel shall wait for their Swab test result (RT-PCR), and they can resume work once they will received a NEGATIVE RESULT. Ifadvised by the Barangay Health Center to not undergo swab test (RT-PCR test) they must also secure a Medical Clearance from their Barangay Health Center as basis for the release of their WFH Medical Certificate. > Ifamember of the same household (living with) of the DepEd employee was texted and swabbed: The DepEd employee is also advised to WFH while waiting for the result. The personnel will resume work once household member has a NEGATIVE result. Personnel: Senior Citizen, Pregnant and with Comorbidities Additional Attachment for DTR: © For individuals to be issued a Work from Home (WFH) Medical Certificate the following must be presented to their respective District / Cluster Nurse: > Pregnant: © Medical Certificate and latest Ultrasound > For New Onset Comorbidities: © Submit Medical Certificate from Attending Physi prescriptions. NOTE: = School Head shall assign task for work from home arrangement and ensure that that the Work Week Plan (WWP) and the Output of the Teacher in their Individual Weekly Accomplishment Report (IWAR) is completed. = Work from Home Medical Certificate will be issued by the School Health Section through their District or Cluster Nurse, which is valid for 3 months (from the date of issuance). * Original Copy Shown signed by the School Head and attached to the DTR can be used and attached only for the 3 consecutive months, ian, laboratory work-ups and B.
