Sub: Requesting for Issuing Certificate mentioning the reason for not properly submitting online VAT return
due to space limit of VAT Portal against SQUARE Hospitals Ltd.

Dear Sir,

We would like to draw your kind attention to the above subject that SQUARE Hospitals Ltd. uses various
Reagent, Chemicals, Capital Machinery, Medical equipment, Spare Parts , consumables etc. which have to
import from various countries.

As per SRO No113-Act/2021/02/Customs, dated May 24, 2021, SQUARE Hospitals Ltd. is qualified to avail
VAT exemption facilities at the import stage but due to no Online VAT Return or not showing the VAT
return documents on the website of VAT Commissionerate, SQUARE Hospitals Ltd. is not getting the same
facilities.

In this regard, we are depositing regularly our VAT amount to the Govt. Revenue Account through the TR
Challan and also submitting VAT return vide Mushak-9.1 on due time to our circle office. Please be
informed that we are submitting our return (Mushak-9.1) manually instated of online due to unable to
upload the challan properly as restriction of upload space limit of National VAT Portal.

We are humbly requesting you to upload the VAT documents on Your Website & also to issue a certificate
mentioning the reason why we are unable to upload the Challans due to space limit of VAT Portal, which
we need to submit to Dhaka Customs House to avail the exemption facilities.
