AFFIDAVIT

I, APRIL ALLEN M. SAYDE, single, of legal age, a Filipino citizen and a

resident of Allen, Northern Samar, after having been duly sworn to in
accordance with law do hereby depose and say:

1. That I work at the Provincial Disaster Risk Reduction and

Management Office (PDRRMO) as a Local Disaster Risk Reduction and
Management Officer II (LDRRM Officer II);

2. That we were quarantined in the University of Eastern Philippines

(UEP) White Beach Resort starting on July 1, 2020;

3. That we were quarantined there because we were frontliners assigned

in facilitating the return of Local Stranded Individuals (LSIs) back
here in the Province of Northern Samar;

4. That I have no personal knowledge on whether some of my co-workers

went outside the quarantine area because I was always inside my
room and I had a separate room because I was the only female in the
group;
