SALAY SAY NG REKLAMO

Ako si Rose Yu Chua, 45 taong gulang, may asawa, residente ng Brgy Patar,
Tagudin, Ilocos, matapos makapanumpa na naayon sa ating umiiral na saligang batas ay
nagsasaad ng gaya ng mga sumusunod;
Na gusto kong ireklamo ang aking asawang si Efren Chua dahil sa mga pang
aabuso niya sa akin. Na simula noong taong 2016 hanggang sa kasalukuyan ay hindi na
ako binibigyan ng panggastos ng aking asawa para sa aming bahay at sa aming 9 na
taong gulang na anak.

Na kapag kinakausap ko ang aking asawa tungkol sa panggastos sa aming bahay

ay sinasagot lang niya ako na wala pa daw yung taong nagpapasuweldo sa kanila at kung
ano ano pang dahilan, ni pambaon ng anak niya ay hindi niya maibigay umaasa na lang
po ako sa naiaabot na tulong ng aking mga anak na may trabaho na.

Na matagal ko na din pong nababalitaan na may kinalolokohang babae ang aking

asawa na nagngangalang Rina San Pedro pero hindi ko na lang po masyadong pinansin
kahit nasasaktan ako.
Na September 10, 2017 kinausap ko na din si Mona tungkol sa relasyon nila ng aking
asawa at nangako siya sa akin na hihiwalayan niya na ito ngunit muli ay nakita silang
magkasama at magkaakbay sa isang inuman. Noong October 2020 ako ay tinetext na ng
kanyang babae na si Rina tungkol sa pag mamahahalan daw nila ng aking asawa at hindi
na raw ako mahal ng asawa ko at iba pa po na nakakasakit ng damdamin ko. Noong
Nobyembre 2020ay kinausap ko ang aking asawa tungkol dito ay inuunahan niya ako ng
galit at minura niya ako at ibinato niya ang electric fan sa akin mabuti na lamang at
nakaiwas ako.
Na nito lamang pong Enero 15, 2021 ay nagsumbong sa akin ang isa kong anak na
lalaki na nakita niya ang papa niya kasama na naman itong si Rina San Pedro
magkaangkas sa motor at nakayakap ang babae sa kanyang ama.
Na paulit ulit na lamang po ang pang –aabuso sa akin ng aking asawa at matagal ko
na ding tinitiis ito. Hindi na nga siya nagbibigay ng panggastos sa akin katulong pa niya
ang kanyang babae sa pagbibigay ng sama ng loob sa akin kaya naisipan ko na silang
sampahan ng reklamo.

Kusang loob kong ibinibigay ang sinumpaang salaysay na ito upang gamitin sa
pagsampa ng kasong Violation of RA 9262 laban kina Efren Chua, 46 taong gulang at
Rina San Pedro parehong residente ng Brgy Patar,Tagudin,Ilocos.

SA KATUNAYAN NG LAHAT ay lalagda ako ngayong ika 10 Hulyo 2021 dito sa

Lungsod Ng San Fernando, Pampanga.

INCIDENT REPORT
NATURE OF THE CASE : Violation of RA 9262

TIME/DATE/PLACE OF INCIDENT: Feb 2020 and September 2021

Brgy. Patar,Tagudin,Ilocos.

FACTS OF THE CASE:

On February 8, 2018 around 1:30 in the afternoon Rose Chua, 45 years old,
married and a resident of Brgy Patar, Tagudin, Ilocos personally appeared to this office
and reported the abovecited incident.

Initial investigation revealed as per statement given by the victim that her husband
Efren Chua failed to give them financial support to her and her 9 year old daughter since
February 2016 up to present. That when victim spoke with her husband regarding said
matter, he just reasoned out that the person in charge of their salary is not yet available
and other reasons.

That victim Rose heard rumors about her husband having an affair with Rina San
Pedro but ignored it, but on Sept 15, 2020 she talk with Rina regarding her relationship
with her husband, and Rina promised that she will part ways with him. But again, victim
saw the two of them intimate together in a drinking spree. On October 2020 victim receive
a text message from Rina telling her about how she and Efren love each other and other
things that hurt the victim’s feeling. Rose speak this out to her husband but her husband
got mad and cursed her, he also throw the electric fan towards her but victim avoided it.

On January 5, 2021, Victim’s son told her that he saw his father with Rina San Pedro
hugging his father while riding a motorcycle.
