Home Quarantine Care Kit
In case you have tested positive for COVID-19 and have mild symptoms and/or are
asymptomatic, there is a possibility that your medical officer might prescribe
treatment via home quarantine instead of hospitalization

In such an event, you can avail a reimbursement for the Home Quarantine Care,
based on actuals, up to a maximum of INR 30,000. The reimbursement amount will
be subject to tax deduction as per the applicable tax slab.

Reimbursement will be done via Payroll. To raise a claim, please fill the attached
reimbursement form and send it to payroll.india@nxp.com along with the COVID
test results* (from authorized hospitals/diagnostic centers) and original invoice/
bills for the home care, original prescriptions by authorized doctor/physician. The
claim can be raised within 60 days of the date of the positive test report

COVID-19 test accepted for reimbursement :

1. RT-PCR Test
2. CORAD score 4 or above
3. CT severity more than 5

This benefit is for employees and their declared dependents

Hospitalization

In case of hospitalization, your Group Medical Policy will provide the required
coverage for you and dependents. The usual process of insurance claim will be
followed for COVID-19 hospitalization
