A. Self-learning Modules (SLMs) Data Entry


1. Encoder will follow same instructions in filling-out the template based from the previous Progress Monitoring on Self-Learning Modules (SLMs).
Provide data for both SLM Printing & Distribution for Centrally & Locally managed. Limit data entry to cells not shaded with colors, shaded cells are with formula, do not remove or replace the content
2. Bayanihan Act 1 - indicates the number of SLMs produced using the re-aligned funds allotted for SLMs (printing and delivery) in support to RA No. 11469 (Bayanihan to Heal As One Act).
3. Bayanihan Act 2 - indicates the number of SLMs produced using the re-aligned funds allotted for SLMs (printing and delivery) in support to RA No. 11494 (Bayanihan to Recover As One Act).
4. Please ensure the value/data that will be encoded in each cell. Some of the cells contains formula that may change the template needed data.
5. Provide the date of data entry.
6. Do not make any changes in the given template. You just need to encode the needed data/value.

B. Learning Support Aides (LSAs), Teacher, Non-Teaching and Related Teaching (NT/RT) Personnel Hired Data Entry
1. Learning Support Aide (LSA) refers to a person who works together in collaboration with a teacher and contributes to the provision of learning opportunities that promote achievement and progression of learners. Source: DO 32, s. 2020
2. Displaced personnel - workers whose employment permanently terminated due to authorized and/or other causes that is related to COVID-19 , as assessed by the DOLE Regional Office.
a. Teaching personnel - personnel in elementary, secondary and tertiary institutions whose appointment or contract of employment indicates a teaching position or faculty rank.
b. Non-teaching personnel - personnel in elementary, secondary and tertiary institutions whose appointment or contract of employment indicates a non-teaching position.
c. Related-teaching personnel - personnel in elementary, secondary and tertiary institutions whose appointment or contract of employment indicates a related-teaching position.

C. School-Based Feeding Program Data Entry


1. Template is divided based on the funding year.
2. For nutritious food, it is divided by the status of procurement, status of completed feeding, and status learners completed feeding cycle.
3. For milk, it is divided by the status of procurement, status of distribution, and status of learners' completed milk feeding cycle.
4. Please ensure the value/data that will be encoded in each cell. Some of the cells contains formula that may change the template needed data.
5. Provide the date of data entry.

Instructions & Do's and Don’t's


1) Look for the specific sheet/tab of your region (Column A).
2) Regional Consolidator must encode data per SDO/Division for uniformity and easy consolidation (Column B).
3) Select from the drop-down list (Column B) the specific SDO with donation or contribution from external partners.
Note: You may use more than 1 row for every SDO depending on the number of resources received.
4) Identify from the drop-down list (Column D) where the donation/contribution came from:
a Barangay/
LGU and other political leaders
b Community Leaders
c Other Government
organizations/Agencies
d Broadcasting/

e Telecommunications Network

5) Regional Consolidator must encode the specific name of the partner who donated the resources (Column E).
6) Identify the following from the drop-down list (Column F) the name of a person/group/organization who contributed resources:

8) Regional Consolidator must encode in the Column H the number of pieces of the donated resources.
9) If there's an amount donated in cold/hard cash, encode the amount in Column I in absolute value
10) If there are non-monetary contributions, convert it to peso equivalent based from the current market value/price and encode it in Column J.
11) The total financial column will automatically compute the sum of monetary and non-monetary contributions/donations (column K).
12) Encode the physical description of the donated resources (Column L).
13) If there's any remark that is deemed useful for the review and consolidation of this report, please encode it in Column M.
14) Using the drop-down list, select the submission number based on the cut-off date of the report (Column C).
15) The google sheet will automatically save your input as long as you are connected to a stable internet line.
