CONTRACT OF SERVICE

Be it known that MARISSA B. VILLARUBIA of Greenhills, San Fernando, Cebu as the Party of Greenhills
Elementary School and Department of Education, IPHO Bldg., Lahug, Cebu City, as the second Party
representing the Department of Education do hereby agree to the following:

A. Reponsibilities of the First Party

1. Secure and safeguard the school premises, school equipments and facilities
2. Assist the pupils in cleaning the surroundings and maintain the landscaping of the school
3. Regulate the entry of non-student & non-teaching personnel
4. Submit to the school Administrator Form 48 at the end of every month

B. Reponsibilities of the Second Party- Department of Education

1. Supervise the security personnel to ensure that the safety and orderliness in the school
will be maintained and perform other duties and functions related to safety and
orderliness of the school.
2. Provide the necessary support to the security personnel in the performance of his duties
3. Certifies attendance and payment of the security personnel

For and consideration of this contract, the Division shall pay from the monthly MOOE of the school:

1. Two thousand Five Hundred (P2,500.00) per month honorarium for ten months:

In WITNESS WHEREOF, the parties hereto have signed this Contract of Service on the 1 st day of
AUGUST 2019 at Greenhills Elementary School, Greenhills, San Fernando, Cebu,Philippines.
