Dear Director General:

In connection to our initial application of License to Operate for DELTAPLUS PHILIPPINES INC as a
distributor of Medical Devices (i.e Personal Protective Equipment), we regret to inform that we are
unable to submit the required QPIRA Training Certificate due to the following reason/s:

We are not yet enrolled on a schedule for the training seminar.

In view of the above, we would like to request your consideration in accepting our application with a
promise to enroll in the training seminar by the nearest available schedule in NCR and submit a copy of
said certificate within a month from the date such certificate is received. We thoroughly understand that
failure to do so shall entail the revocation of our LTO.
