CERTIFICATION
This is to certify that permit is hereby given to DepEd Division of Samar to conduct Project
PAGDUAWA on January 6, 2021 at Motiong National Highschool Cultural Center with a maximum
of Seventy Three (73) Participants. As such, the Division Superintendent thru its District Head,
should make sure that all Health Protocols / Minimum Public Health Standards or the “BIDA
Solusyon Strategies sa COVID-19” should be followed.

This permit is anchored on the Advisory of the DILG dated December 30, 2020; the
subsequent letter from the OIC-Director IV of DOH-EVCH Exuperia B. Sabalberino, MD, MPH,
CESe dated December 31, 2020; the OMNIBUS GUIDELINES ON THE IMPLEMENTATION OF
COMMUNITY QUARANTINE IN THE PHILIPPINES with Amendments dated December 14, 2020,
Section 5, Item 5; and upon the recommendation of the Motiong Inter-Agency Task Force
(MIATF).

This Certification is issued to Carmela R. Tamayo, EdD, CESO V, Schools Division

Superintendent upon the request of Marina Muriel Y. Labid, Ph.D., District Head, Schools District
of Motiong.
