I am applying to medical school this summer and am writing to ask if you would be willing to
write a strong letter of recommendation in support of my application.

[Brief sentences about your involvement with their class/organization, time period, what you
enjoyed or learned]

Should you decide to recommend me, I will provide my CV, personal statement, transcript,
and any other information you would find helpful.

It would be great to meet in person to discuss, your schedule permitting. Please let me
know if this is at all possible. Thank you so much for your consideration, and I look forward to
hearing from you.

Instructions
Read our Letter of Recommendation Guide for further explanation. It includes who to ask,
when to ask, what to provide, common mistakes to avoid, and FAQs.

Adjust and adapt the template above as needed. Remember, your letter writer is probably
very busy and appreciates brevity and conciseness—get straight to the point politely
without wasting their time.

When you email them, cc their administrative assistant, if applicable. If they agree to
meeting in person, provide several time slots and be flexible—again, make this easy for
them.

If they agree to writing a strong letter of recommendation, provide them with the following:

1. Updated CV
2. Personal statement

4. MCAT printout (optional)
5. Personal blurb (optional)
6. Submission information, including your AAMC ID and a unique Letter ID

In your follow up email, provide them with the deadline for which they should submit their
letter of recommendation. Set a reminder for yourself to send them a follow up email 2
weeks prior to the deadline.
