ACCOMPLISHMENT REPORT
For the Month of October, 2018

 The Teacher attended Department meeting.

 Students performed activity for better understanding.
 Teacher used Multimedia presentation to present her lesson
 The made lesson plan daily and prepare LSM congruent to her lesson.
 Have beautified the lawn assignment.
 Have structured the room and make bulletin board updated.
 Have passed documents required of her.
 Have attended the Inservice training and have served as support staff.
 Have prepared the TOS and Test questions for the second periodic test
 Have conducted the second periodic test to all sections handled.
 Have initiated project related to recycling.
 Have participated in the Municipal celebration of the teachers’ day.
 Have joined the parade and the night activity which is the DepEd Night during
the town fiesta celebration.
