Be it ORDAINED by the Sangguniang Bayan of Agoo, La Union that ARTICLE I Title of the Ordinance SECTION | Title of the Ordinance. ‘This ordinance shall be known as the New Agoo Public Cemetery Code of Agoo, La Union. ARTICLE IL Definition of Terms SECTION 2. As used in this Ordinance, the following terms shall mean: Burial — interment of remains in a burial chamber Burial Chamber — apartment type concrete niche intended to accommodate cadavers inside coffins or other containers, Burial Grounds ~ cemetery, memorial park or arty place duly authorized by law for permanent disposal of the dead. Candelabrum — an area where individuals can offer candles und prayers for the dead E. Common interment Area — a common bone depository for bones not being transferred within 5 years of interment Cremation ~ to incinerate a corpse Death Certificate a document issued by the attending physician orn his \ absence by the Municipal Health Officer or other duly authorized government official as cited in Section 14 of this ordinance, using. the ~ prescribed form certifying the death of person Hi. Disinterment — the removal or exhumation of remains from places of ‘interment 1 Ecumenical Chapel ~ a memorial chapel used for the special services of the dead prior to imerment. A place to hokl mass and prayers in the cemetery J. Health Certificate — a certification in writing issued by the Municipal Health Officer using the prescribed form to & person employed in the funeral establishment and other related services after passing the required physical and medical examinations and immunizations K_ Immediate Family — shall include the father, mother, brother/s, sister/s, sons, daughter's, husband, wife of the deceased L._ Indoor Repository Niche ~ an interment space of cremated cadaver placed in a vault of we on eng, Mel Forapail? Maimenance Fee — a fee paid by the family of the deceased for the general upkeep of the chamber and the cemetery as a whole that includes painting/repainting, cleanliness and orderliness, security, lighting and & Personnel. S52 and 856 and local ‘ordinances U_ Sanitation Inspector — a government offices, employed by the Municipal Government who implements environmental sanitation activities under the supervision of the Municipal Health Officer: V. Location, The New Agoo Public Cemetery is located at Barangay San Antonio, Agoo, La Union and other areas as may be determined by the local government. Five (5) blocks for children burial chamber with 169 lots (3 and 4 levels), S68 units A ossuary niche marked A-E with 2, 176 Jots (+ levels) and 10.864 units Indoor repository niche with 30 fots (4 levels) with 120 urn vault units. The maps of the New Agoo Public Cemetery in the Municipality of Agoo shall be made integral part of this ordinance. ARTICLE IV Rules in the Disposition and Use of Burial Chamber BALDERAS son set SECTION 5. Block 1, Block 2 and Block | 1 will be designated for reservation of families who wish to use a burial chamber other than those located inthe perimeter fence. Reservation atthe Ossury Niche and Repository Niche can 2 ay also be availed. Burial chamber will be leased for 5 years after payment of the corresponding fee. Said burial chamber shall be disposed in sequential order following the serial numbers on the chamber. : Burial Chamber will be used for 5 years only. At the end of the 5-year period, the remains of those interred therein shall automatically be removed and transferred to the niches for bones upon payment of the required fee Otherwise, the remains shall be transferred to the common interment area and in said case, the exhumation cost will be shouldered by the Municipal Government SANDRA Y. Automatic transfer of bones after five years of internment All buried cadavers shall automatically be transferred to the ossuary niche designated ‘area in the cemetery upon payment of the prescribed fees at the expense of the family. If relatives fail to twansfer the remains within the prescribed period. the Municipal Government through its designated staff will automatically taz to identify the remains and transfer the same to the “common interment area” a FLROENT O. A list of deceased persons from the burial chamber subject for transfer will be posted at the Cemetery Administration Office, infront of the Municipal Hall, Civic Center and Agoo Hypermarket. The disinterment or exhumation permit shall be issued by the Municipal Health Officer after payment of the required fees and all disinterment of remains shall be under his/her supervision. A. Requirements for the disinterment or exhumation of remains of persons who died of other than dangerous communicable disease: 1. Permission to disinter the bodies or remains of persons who died of other than dangerous communicable disease may be granted after > such bodies have been buried for a period of five (5) years. “The remains upon exhumation shall be disinfected and placed in a” sealed coffin case or bowermtically sealed container, and properly identified as to the name of the deceased, date and cause of death and place of reinterment 3. After the 5-year closure order of the old cemetery. it shall be open for use following the New Agoo Public Cemetery Plan of the LGU. ARTICLE XV Penal Provision Z SECTION 35. Penal Provision.
