Transmitting herewith for your endorsement and approval Variation Order (Extra Work/Change) No. 2
for Concreting of Purok Sansapan – Abtois – Purok 1 FMR, Barangay Upper Labay, General Santos City
and funded through the ___________________________ of the Department of Agriculture and
LGU General Santos City. The said project with a contract cost of PhP19,260,740.34 as per approved
variation order no. 1 is being undertaken by contract through MJ Tiongson Construction.

The said Variation Order (Extra Work/Change) No. 2 as evaluated and recommended for approval by the
Technical Review Team _______________ meeting in Isulan, Sultan Kudarat comprised the increase
and decrease in quantities of some item of the original program, including the introduction of a new
work item necessary for the improvement and protection of the project to wit:

Sample list:
1. Deduction of quantities for Items 102 (roadway excavation), 105 (subgrade preparation), 201
(aggregate base course) and SPL1 (backfill) from sta 1+685.70 – Sta 1+767.30 due to restationing
of the completed DPWH road project.
2. Introduction of new Item 101-1 (Removal of Existing Pavement) and Increase in quantity for Items
311 (PCCP) for works to be undertaken at Sta 1+282.661 to Sta 1+371.361
3. Increase in quantity for Item 505 (riprap & grouted riprap).

Included also in the conditions of the aforecited Variation Order No. 2 are the additional costing of
PhP346,088.62 and a time duration of _______________ calendar days to complete the said extra
works. The additional cost will be taken from the generated savings in the procurement of the original
program.
