1. Submit this form to your Unit Timekeeper as follows:
- Every Monday of the following week - for OT services rendered from Monday to Sunday of the previous week.
- 1st banking day of the following month - for OT services rendered during the last remaining day(s) of the previous month
2. OT services rendered in meritorious cases when employee's non-compliance with attendance requirements for overtime as may be authorized by the Dept/Unit Head whenever necessary,
provided appropriate certification/justification by the immediate Supervisor is secured.
3. Group Head approval shall be required if the OT rendered for the month exceeds the 60 hours limit.
