Requirements for New Application

1. Letter request address to EMB Regional Director (ENGR. MARIA DORICA NAZ-HIPE)
2. Duly accomplished application form

3. Engineering Report prepared, signed and sealed by a Mechanical Engineer with Photocopy
of PRC ID and Managing Head/ Owner and duly notarized.(w/initials every pages of report)
· Description of processes including the amount of raw materials and finished products,
all emissions into the atmosphere and/or material balance.
· Process Flow Diagram indicating all possible sources of air emission.
· List of Air Pollution Sources Installations (APSI) and its corresponding Air Pollution
Control Facilities (APCF) with complete specifications.
· Discussion of each APSI showing the process/mode of operations, design criteria and
efficiencies of the APCF including the types and quantities of pollutants emitted.

4. Plant Layout and specifications duly signed and sealed by PME

· Plans and Specifications of the proposed Air Pollution Source and Control Installations
(APSCI)
· Plant and Machinery layout showing the connection of the APSI to the APCF.

Note: For stand-by Generator Sets with less than 300 kW / capacity submit copy of
manual/picture of the name plate of engine and alternator (specifically nameplate of
engine and alternator)
5. Copy of DTI Certificate/ SEC Certificate
6. Vicinity map identifying the address, location of the plant premises
Geo-Tagged Pictures of the Site and Equipment
Site Development Plan
7. PCO designation signed by the Managing Head/ Copy of Certificate of PCO Accreditation
8. Copy of ECC/CNC
9. Filing Fee : P640.00
