Case Study: Pesticide poisoning

A 30-year-old, comatose male with miosis, diaphoresis, and incontinence

You are alerted that a 30-year-old, unconscious male worker passed out. While on duty, the

worker suddenly developed headache, dizziness, weakness, nausea, vomiting, and diarrhea. Few

minutes after you attend the situation, he lost consciousness and experienced urinary and fecal

incontinence. Also, you note that he has fixed pinpoint pupils, generalized paralysis, fasciculations,

and is unresponsive to deep pain. You call for an ambulance and continue evaluating the situation.

He has profuse salivation, diaphoresis, and excess lacrimation. Paramedics give first aid and

transport the patient to the medical facilities. Vital signs include the following: blood pressure

140/90 mm Hg, temperature (36.1°C), pulse 58 beats/minute and regular. In the emergency room

mucoid secretions are suctioned from the trachea at the time of intubation, and mechanical

respiratory support is instituted. After clinical manifestation evaluation and laboratory test, doctors

determine pesticide poisoning.


From the patient’s workmate, you learn that he started working in the company yesterday. This

morning he changed into work clothes and began mixing pesticides for subsequent tree spraying

in the company green areas. About noon, the patient became nauseated and started sweating

profusely.

area yesterday. The patient had been mixing several chemicals and spilled the mixture on the floor.

After cleaning the spill with his shirt, he stored his working clothing in the looker. Then he

showered and wearied his street clothes. He saw no reason to mention the incident to his boss.
