Past history:

He started taking drugs at very early age when he was 10 years old. His father was died before
his birth and after his mother’s death he used to take drugs continuously. He got very disturbed
on his mother’s death and used to recall the time he spent with her mother. Most of the time he
felt depressed and mean time he himself tried to rid them off. But from past 3 years he is
addicted of heroin. And his behaviour got changed with his family. He fought with his sisters and
brothers. His behaviour was very aggressive.

Present history:

He is addicted of heroin and cocaine. He used to take these drugs to get relief from his
depression. He wasn’t forced by anyone to take these drugs. It was his own choice because he is
suffering from depression too. He said that he used to miss his mother a lot. He thought that he
didn’t give their parents good time. He wants to have his parents back. Just because of this all
things he is suffering from depression and used to take drugs to get relief from all these thoughts.
Then my client finally came to fountain house for his treatment with the referral of his friend.
And now he’s very much stable person.

Family history:

My client is belongs to middle class family. He is 8 siblings, 6 sisters and 2 brothers. His father
was died just before his birth. His mother was house wife and died in 2010.
His birth order is 8th in his siblings; his relationship with his family was good. He was very
much attached to his mother and sister’s.

Family history of psychiatry:

My client has family history with addiction. His brother was used to take drugs.

Family history of physical:

My client family is physical healthy. There is no physical illness.

Birth:

The client had normal birth. There were no complications to his birth. He had passed all the
developmental mile stones normally and at appropriate age.

Sexual and marital history:

My client is married.

Pre morbid history:

Before was active and had good health. His behaviour with his friends was good. He has social
gathering. He likes to listen music. Before using drugs he had no bad habit but sometimes he
fought with the people around him. He didn’t like to share things with his siblings.

Mental state examination

 Appearance / behaviour:

He was wear shalwar kameez and slippers. His hair was short and combed, his shave had
done, his gait was normal. His behaviour was cooperative and felt comfortable during the
session. His facial expression was happy and sad depend upon his mood and his eye contact was
fixed.

 Speech:

Form: His tone was sharp and organized, he talked actively.


Content: His talk was relevant to the topic. He answered my question according the topic.

 Mood:

Objective Mood: His mood was normal but sometimes he feels depression.

Subjective Mood: He said that his mood is stable. He was sad on his attitude toward his
family.

 Thought:

Form: He has depression like thoughts.

Content: He was involved in negative thinking

 Memory:

Digit span: His visual digit span was 4 and auditory digit span is 6.

Short term memory: His short term memory was good.

Long term memory: Long term memory was good.

 Orientation: He was well oriented about time, place and people.

 Insight: He used to think about his addicted behaviour.

 Abstract thinking: His abstract thinking was not so good.

My client’s responses show that he gives importance to his fami1y. In these responses

My client’s response shows that he sometimes enjoying social gathering but in most situations
he does not understand that what others wanted from him. In these responses 19,29,30,31,36
represented social attitude that he was not so much friendly.

 Sexual attitude:

My client’s responses show that he has not interested in sexual relationship. In these responses
20, 23 shows sexual attitude.

 General attitude:

My client’s responses show that he has mixed thinking about the different things. He has guilt of
his addiction and some elements of hopelessness is also exist. These responses
5,6,7,8.9.13,14,15,16,18,21,22,25,28,33,34,35,37,39,38 shows general attitude.

House:

House represents associates concerning home-life Intrafamiliar relationships, attitude toward


their home situation ( children), relationship to parents and siblings and married adults.
According to this drawing this big house represents that client may be completely dominated by
family. Roof represents fantasy life and incomplete roof indicates avoidance of fantasies.
Windows, sideways and door related to openness willingness to interact with others and ideas
about environment. Client drawn open door and windows which indicates his strong need to
engage with others. Lines and walls represent boundaries and strength of ego. Client drawn a
chimni above his house which indicates his good intelligence.

Tree:

Drawing of the tree express the way by which examinee perceives the environment and his
position in the environment/social setup. Trunk represent ego or sense of self. Client drawn small
trunk which reflect limited strength of ego. Branches reflect the effects that our ego make to
“reach out” to world and support “things that feed us” what we need. In this drawing the client
drawn small branches which reflect limited skills to reach life goals. Leaves are the sign to reach
out success. According to the client this tree is apple tree which indicates his high need for
nurturance. He also drawn some birds on tree which shown his power and full ego strength.

Person:

Drawing of a person express the test take’s relationship with other people aside from his family.
Client drawn a picture of a person of opposite sex ( femal ). Arms reflects the way we iteract the
environment and hands the way effect it. He drawn open arms which reflect his willingness to
engage in his environment. Mouth is the way how we get our needs. According to this picture he
drawn a bug smiley face with heavy line and teeth which indicate his verbal aggression toward
others. Legs and feet represent power and need for security. Absences of chin indicates
compensation of weakness. Too much focus on hair by client indicates critical personality and
too much sexual deceives. The smile on the face of the person reflects what he do in real life.
