Optimum Mobile: Three free months of Optimum Mobile Service available to new Optimum and
Suddenlink Internet and/or TV customers, and current Optimum Mobile subscribers who add a new
mobile line and add Internet or TV Service. Customer must sign up for Optimum Mobile within 30 days
of Internet or TV service activation or will forfeit credit. During three month promo period, customer
may select any Optimum Mobile data plan and upgrade/downgrade at any time. After promo period,
customer will be charged for their Optimum Mobile Service plan at our then standard rates. Activation
fee for Optimum Mobile Service will be waived. International calling and data plan not included.
Standard taxes and fees apply. Credit will be applied in the second bill cycle after Internet or TV service
activation. If Internet or TV service is discontinued for any reason, Customer will lose the free offer.
