COMPLAINT-AFFIDAVIT
Complainant, under oath, states that:

1. I am currently employed at Bureau of Fire Protection (BFP) as

Fire Officer I, assigned at Santiago City Fire Station, Santiago
City, and I personally know S/Insp. MANUEL GIRAY GOLINO
who is at present the Fire City Marshal of Santiago City;

2. The afore-named BFP is legally married to one LILIBETH DE

LEON-GOLINO of Aparri, Cagayan and from their wedlock, the
following children were born, namely: Liniel Marie Nino Golino
and Ligui Marvis Noel Golino who were born on April 1, 2002
and February 5, 2004, respectively;

3. Despite the fact that the afore-named BFP officer is legally

married to Lilibeth De leon-Golino and is known publicly as
married to the said person, he is maintaining not only one illicit
relationship but at least three (3) illicit relationships with the
following women, namely; MARIVIC “BING” REYES of Difun,
Quirino; JULIE ANN BASCOS AGCAOILI of Patul, Santiago City;
and MARITES UMAGUING of Rizal Santiago City;

4. In fact, the illicit relationship of the afore-named BFP officer

with Marivic “Bing” Reyes resulted to the birth of two children,
namely LEMUEL and NINAY;

5. The afore-mention BFP officer is maintaining his illicit

relationships with the afore-named women openly and publicly
in utter disregard to the decency and moral standards for
government employees;

6. In fact, adding disgrace and shame to his act of openly

keeping and maintaining these illicit relationships with the
afore-named women within the knowledge of BFP employees, the
afore-named officer allows these women to sleep at the fire
 station in the full view and knowledge of my fellow BFP
employees;

7. I have personal knowledge of the foregoing shameless,

immoral, and abominable conduct of the afore-named BFP
officer because he does not keep the same as secret to the
employees of the BFP and he has in fact introduced these
women to be his mistresses or girlfriends when in fact he is very
much married man;

8. I am executing this affidavit to attest the truthfulness of the

foregoing facts and for the purpose of filing administrative
charges against S/Insp. MANUEL G. GOLINO for Grave
Misconduct;

9. For the purpose of complying with standing rules and

procedures in administrative cases, I hereby declare:

a) I have not instituted similar complaint against the

afore-named person arising from the same facts and
circumstances before other administrative tribunal or
agency;

b) No similar administrative complaint is pending before

any administrative tribunal;

c) Should I learn that a similar complaint is filed or

pending against the afore-named in another tribunal, I
undertake to notify this administrative tribunal handling
this case within five (5) days from knowledge thereof.
