Employees Aadhar card and employee ID to be carried
Prior to
vaccination A mask that covers nose and mouth and fits tightly and comfortably

Half sleeve and loose t-shirt to be worn on vaccination day

Carry a screenshot or print out of time slot given by corporate SPOC

Carry a note of previous health issues, allergies or medicines being


consumed currently

Avoid getting vaccinated if you observe any of the exclusion criteria

Keep your mask ON at all times and don't touch your mask once it's properly fitted

Person with a history of anaphylactic or allergic reaction to a previous dose

Prior to Pregnant women or lactating mothers.

Vaccination is to be deferred for 4-8 weeks in patients who have had SARS-
(3/3) COV2 infection.

Acutely unwell persons with any illness.

Persons who have a coagulation disorder.

Persons who are on oral anticoagulants or Low molecular weight Heparin.

Blood pressure more than 180/100 mm hg . Advise to get the vaccination in


the hospital
