The Privatization and Management Office (PMO) is a government agency organized

under the Department of Finance pursuant to Executive Order No. 323 dated 6 December 2000.
It is tasked to take title to and possession of, conserve, provisionally manage and dispose of
assets of the National Government, government corporations and other properties. Among these
assets are the properties registered in the name of:

National Power Corporation covered by TCT No. (458365) S - 77242

In this connection, we respectfully request for certified true copies of Tax Declarations
that may have been issued by your good office in relation to the aforementioned title.

John Robert O. Bautista with PMO I.D. No. 244 is authorized to submit this request, to
follow up on the issuance of the requested documents and to receive such documents on behalf
of PMO.
