AFFIDAVIT OF CLOSURE

I, SHERWIN DELA RAMA, of legal age, Filipino, with residence address at

1213 Morong St., Malinta, Valenzuela City, after having been duly sworn to in
accordance with law hereby depose and state:

1. That I am the registered owner of business name TRIPLE- S Bangus Dealer with
business address at Stall No. BB-12 Farmers Market, Cubao, Quezon City;

2. That the above business has already ceased its operation since March 31, 2019
due to business loses severe financial reverses;

3. Hence, I am applying for the termination of the above business effective

immediately upon filing application/affidavit of closure of business;

4. That I hereby certify that upon filing the application for termination of the above
business, the same has no financial liability or obligation to any third person;

5. That I am executing this affidavit to attest to the truth of the foregoing and for
whatever legal purpose/s it may serve.
