AFFIDAVIT OF GROSS SALE

I, ANASTACIO M. REGULTO, of legal age, Filipino, with residence and postal

address at Block 21, Lot 11, P2 Ext. Mabuhay City, Brgy. Mamatid, City of Cabuyao,
Laguna, after having been duly sworn to in accordance with law hereby depose and
state:

That I am the true and lawful owner of TACING’S BAKERY with business
address at Block 21, Lot 11, P2 Ext. Mabuhay City, Brgy. Mamatid, City of Cabuyao,
Laguna;

That the said business establishment gain gross sale in the amount of Two
Hundred Thousand Pesos (Php.200,000.00) Philippine Currency, from January
2020 up December 2020;

That I hereby declare under penalties of perjury that the forgoing attestations are
true and correct;

That I am executing this affidavit to attest to the truthfulness of the foregoing

facts and for any legal purposes that may legally serve.
