A confirmation email will be sent to your REGISTERED EMAIL after you completely submitted the
Digital Registration.

While email regarding the Zoom Details, Evaluation Link, Reminders for the Convention will be sent to
your email at least 1 week before the date of the actual Convention.

Please make sure you are using a WORKING EMAIL ADDRESS and double check your email before
submission of Registration Form. If you do not receive a confirmation email, kindly check your SPAM
Messages.

Kindly prepare a Screen Shot of your proof of payment to be attached at the Registration Link.

POLICIES ON REGISTRATION:

1. A clear copy of proof of payment should be attached to the digital registration form upon
registration. it should be in a JPEG or pdf format with file size not bigger than 10 MB.
2. Use the Provided Link For Online Registration.
3. Paid registration is transferable upon presentation of authority to transfer.
4. Official Receipt:
✓ Official Receipt will be sent via payee’s registered EMAIL address after bank
verification of at least (5) days.
✓ Original Copy of Official Receipt is available upon request of the participant/s and
will be sent via LBC with the mailing fee charged to the participant.

5. Continuing Professional Development: Trainees and Participants are entitled to

Certification of Participation with corresponding CPD points
a. Certificate of Participation will be sent to individual email address at least 10-15
working days after the Convention.
b. To receive the Certificate of Participation, the Trainees/Participants should:
i. Convention Attendance (Completed)
ii. Convention Digital Evaluation (Completed)
iii. Registration fee (Fully paid)
