AFFIDAVIT OF CONSENT

We, JUAN DELA CRUZ and JANE DELA CRUZ, both of legal age, married and
residents of Davao City, Philippines, after having been sworn in accordance with law,
hereby depose and say:

1. .1. That we are the parents of PEDRO DELA CRUZ, who was born on April 27,
2013 in Davao City, Philippines;

2. That we are giving our consent for our child to travel abroad to be accompanied
by his paternal grandfather, JAMES DELA CRUZ;

3. That we are sponsoring his trip;

4. That we are financially capable to support his stay abroad;

5. That we guarantee that he will not violate any existing laws of Dubai; and

6. That we are executing this affidavit in connection with our child application for a
passport at the Department of Foreign Affairs and/or travel clearance from the
Department of Social Welfare and Development for her travel abroad.
