Regards & Satety sia Interriational (China) Limited Partnership ee REPORT ON NEAR MISS INCIDENT ON VESSEL ‘A’ WITH FISHING VESSEL ‘B’ WHILE EMBARKING PILOT IN BRIXHAM INCIDENT The Panamax bulker ‘A’ was proceeding to Brizham to pick up the North Sea Piot prior proceeding to Amsterdam to discharge her cargo of coal. While picking up pilot, the vessel 'A’ had @ near miss incicent with fishing vessel "6" The fishing vessel subsequently filed a report with the Coast Guard on the near miss incident, DETAILS OF THE NEAR MISS INCIDENT (14" February 2000 - all timings in GMT) 1 At 1716 Z, the vessel ‘A’ was 6 miles off Britham piot siation. The Main Engine wwas put on Standby and the ship's speed was 8.0 kts.” Between 1718.2 and 1729 Z, the Mein Engine was tried out. The Main Engine was alzo tried out from the local positon in the E/R. The emargoncy STOP from the bridge was also tried out for the Y2K test. Trafic all around being monitored, only 3 fishing vessels. & 1729 Z - Vessel ‘A’ on a heading of 350 T and speed of 3.2 Kts. The vessel ‘A’ meintaining her course of 380 T and speed of 5.5 kis. The pilot boat is 5 cables on the port bow. The fishing vessel ‘8’ increased speed and tried to cut across the ship's bow. The fishing vessel ‘B’ then panicked and called the vessel ‘A’ and asked her to reduce speed. ‘The Master as per his plan had to swing to starboard and pick up the pilot on the port side and head for the English channel keeping the wind and sea on the starboard quarter. ats » ‘The fishing vessel was answered on the first call, Main Engine put on Stop and wheel put hard to starboard. The vessel swung to starboerd, and the fishing vessel crossed the bow at a distance of 1.2 cables. y AN 2 Bl *\ ky so we 1747 Z__- Brixham Coest Guard informed the vessel thet the fishing vessel was calling the vessel. The Coast Guard was informed that contact had already been established with the fishing vessel and that the vesse's were clear. The fishing vessel ‘B' stil under panic then called the Coast Guard and informed them that the vessel ‘A’ had not given him sufficient clearance. The Coast Guard asked the fishing vessel ‘B’ to calm down and inquired when he first spoke to the vessel. The fishing vessel replied that he had spoken ‘o the vessel ‘A'10 minutes, ago, at which stage the vessel informed the Coast Guard that this was not tue and thet they had spoken just 2 minutes ago. The Coast Guard told the fishing vessel that ihe wanted he could come in the next dey and file a report, The Coasi Guard then asked the vessel for its name, which was given 1748 Z_ - Pilot boat alongside 1750Z_- Pilot on board and vessel proceeding on passage to Amsterdam, CAUSE OF THE NEAR MISS INCIDENT 1 Failure to establish contacts with the fishing vessel and make intentions clear. 2 Increasing speed to Full Ahead at 1731 Z. Si Insufficient distance maintained from fishing vessel. ACTION TO BE TAKEN ‘The following action is to be taken to avoid repeiition of such incidents: When maneuverng and in close proximity of vessels, inform them on VHF of your intentions. The coast guard also picked up the \VHF conversation and informed the fishing vessel he could, if desired, come in and file his report the next day. ‘he vessel did not feel it necessary to inform the office as this was only a ‘near miss" . Subsequently the fishing vessel did file a report with the coastguard. And one day out of the blue we were asked to explain this incident by the owners. You can imagine our embarrassment at not only being unaware of such an incident, but also in that our quality procedures implementation was found severely lacking (_non reporting of near misses) Reporting of near misses and their review on board and in the office is one of the most important tools in avoiding repetition of past mistakes and thus avoiding future accidents, continuous improvement of working standards and enhancement of safety awareness. lis importence cannot be overemphasised, All on board the vessel should be encouraged to report near misses to their superiors. ‘We must learn to earn from our mistakes 1. Please ciscuss the importance of reporting near misses in the next sefety committee mesting. Such a discussion as well as review of the near misses and other incidents should be made a permanent part of the agenda in every meeting. 2. All incident reports (including for the near misses) to be faxed io this office ASAP after the incident. ( A copy to be Sent in the mail as routine) ‘Some vesse's have not reported near misses for long periods..
