Sub: Representation regarding Charges for imposing minor Penalty on me

Sir,

Sorry to inform you that i was left my HQ after I became Covid positive from dt 03.05.2021. I
was unable to submit positive report due to i have tested in private pathology and He refused to given
report. I was self isolated and take medicine. During this period my brother was became very serious due
to suffering from covid positive and admitted in hospital on dt.06.05.2021 and deceased on dated
26.05.2021 my whole family was under mental depression. After completion of work of my late brother
and situation became little normal in my family, I have reported to my HQ on dt 08.07.2021.

So, I request you to please forgive me from the charges imposing minor penalties. Sir, intentionally i
was not detain my self out of Headquarter. I was really unhealthy and became mentally depressed due to
my brother was expired . Sir, I had left my Headquarter for a long period because of above reason and of
this pandemic situation. Sir, I promise you, I will not repeat this type of mistake in future.
