AFFIDAVIT OF DISCREPANCY

I, RHODRI L. LAZARO, of legal age, married, Filipino and a resident of

Brgy. Balabago, Jaro, Iloilo City, do hereby depose and say that:

1. That I am planning to correct my name via Correction of Entries before the

Civil Registry of Caloocan City;

2. That when I secured a copy of my Certificate of Live Birth at the Local Civil
Registrar and Philippine Statistics Authority my name appears to be
IGNACIO L. LAZARO, JR. while in my baptismal documents, scholastic
documents, transcript of records, and other private and public documents,
my name is RHODRI L. LAZARO;

3. That I am executing this affidavit to attest to the truthfulness of the fact that
there is a discrepancy in my name in my Certificate of Live Birth issued by
Philippine Statistics Authority where it is reflected as IGNACIO L. LAZARO,
JR. where in truth and in fact, my true and correct name is RHODRI L.
LAZARO;

4. That the names RHODRI L. LAZARO and IGNACIO L. LAZARO, JR. refers
to one and the same person and that is me;

5. That I am executing this affidavit to attest to the truthfulness of the foregoing

facts and for whatever legal purpose this may serve well.
