Nominal Group Technique - Multivoting Tool

This template guides you through a simple elimination technique that can be used to determine the final, and vital few
causes to a problem.

Basic Steps:
1. Conduct a brainstorming session using the Nominal Group Technique to capture Potential Root Causes (PRCs)

2. List your 12-15 PRCs in the first column.

3. Have each team member assign a score to each PRC on a scale of 1 to 8*.
*8 = Highest degree of correlation between PRC and CTQ; 7 is the second; 6 is the third and so on to 1, or the lowest correlation.
4. Add all the scores from the team members for each particular PRC.
5. Use the resulting Pareto to analyze your results

The PRC with the highest score is ranked 1st, the one with the second highest score 2nd, and so on.
