resident of Block 8 Lot 3 Dreamland Heights, Brgy. United Bayanihan, San Pedro,
Laguna after being duly sworn to in accordance with law, hereby depose and state, THAT:

1. I am the father of Karen G. Villate who was born on Novermber 27,1994 in

Muntinlupa City, Philippines;

2. That I personally know Angelo Neil R. Ang, of legal age, Filipino, son of
Norbert Z. Ang and Thelma R. Ang, residents of NTA Enterprises Bagabag,
Quirino Highway NTA Enterprises, National Highway, Brgy. Quirino, Bagabag,
Nueva Vizcaya 3711.

3. That I acknowledge and know for a fact, that my daughter, Karen G. Villate and
Angelo Neil R. Ang are exclusively in relationship since 2017 and have been
living together from March 5,2018 up to present.

4. That I execute this affidavit for whatever legal purpose it may serve in
connection with my child and her common-law-partner’s application for student
permit and open work permit in Canada, respectively.
