NOTE:

1. For inquiries on the submission of specimen for U/A and S/E, please proceed to the
laboratory department. Sterile containers will be released there.

2. After completing the diagnostics, compile all your official results and proceed to the OPD for
physical exam, which will be performed by the ROD.

3. Employees above 50 years old, include ECG or as per doctor’s request.

4. For your physical exam, please fill up on an OPD sheet together with the medical
examination report. Please secure there forms from the out patient department.

5. Submit all forms to Maam Mitch Bendol in a short, brown envelope as soon as it is
completed.

In our Institution, personal health is of great importance. It is essential that every staff must be
healthy and in good shape to provide effective and efficient health care service and to prevent
transmission of unwanted communicable disease from staff to patient and vice versa. As part of
of VPGHI’s aim in providing the best possible services to our employee, we will be conducting
our Annual Physical Exam. The diagnostics will include Blood Count, Urinalysis, Fecalysis and
Chest X-ray. The aformentioned test will be on January 7-11,2021. Please take note of your
schedule per area attached herewith.

Please take note that thisis mandatory and that all records will serve as file and will be kept in
confidential matter.Compile results and submit in a brown envelope to the billing section c/o
Maam Mitch Bendol. Also, attached herein is a copy of the checklist to which you will need to
complete. Submission will be ONE (1) day after your scheduled test.
