Abstract: Oral submucous fibrosis (OSF) is a premalignant condition caused by betel


chewing. It is very common in Southeast Asia but has started to spread to Europe and North
America. OSF can lead to squamous cell carcinoma, a risk that is further increased by
concomitant tobacco consumption. OSF is characterized by abnormal collagen deposition. It
is a precancerous disorder and transforms into a malignant tumor in 1.5–15% of all cases.
OSF is a diagnosis based on clinical symptoms includes hypovascularity leading to
blanching of the oral mucosa, ulceration, xerostomia, a burning sensation, & restricted
mouth opening and confirmed by histopathology. All of these greatly interfere with patient
quality of life. The present paper introduces OSF from a molecular perspective and
summarizes what is known about its underlying mechanisms, diagnostic biomarkers, and
therapeutic interventions. In addition to the aggressive treatment of OSF, its prevention is
also important.

Approximately 600 million persons are betel chewing, with a hot spot throughout the
Western Pacific basin and South Asia. This makes betel the fourth most-consumed drug after
nicotine, ethanol, and caffeine.
Asian immigrant communities are growing in Western countries. Some authors have
suggested implementation of an oral cancer screening by health care providers. On the other
hand, the concept of screening of an asymptomatic patient is not well understood by many

immigrants. Physicians and dentists in the Western world should know about OSF to make
an early diagnosis that will help to reduce morbidity and mortality.

The Presentation will illustrate the relentless progression of OSF and its significant
morbidity and mortality; they also emphasize the importance of close follow-up of such
cases. Because of the significant cancer risk among these patients, periodic biopsies of
suspicious regions of the oral mucosa are essential for early detection and management of
high-risk oral premalignant lesions and prevention of cancer. Dentists can play an important
role in both the education of patients about the perils of chewing betel quid and in the early
diagnosis of high-risk premalignant lesions and cancer.

College, Darbhanga, Bihar, India, acquired Fellowship in Oral Implantology, Dental Aesthetic
Surgery, Laser Dentistry & Trichology; having credit of 68 Certificates in his Educational Journey;
accomplished 6 researches, 44 Scientific Presentations, 27 Publications in National & International
Journals, authored 3 Chapters and 2 Books for Germany & Nova Biomedicals, New York. He is
Editor of 5 journal house and extensively working on Periodontal Plastic Surgery; Bone
Regenerative Dentistry, Advanced cum Strategic Implant Dentistry & Laser Dentistry, and is widely
involved in academic & research activities.

joined the research group of Prof. James at the Institute of General and Inorganic Chemistry, Bulgarian Academy of
