Karen a 23-year-old woman was rushed to the emergency department of San Pedro Hospital.
Her chief complaints were shortness of breath, headache, nausea, and numbness in her
extremities. She said all of this happened after she took her medication for anxiety that night.
Her blood pressure was 103/55. Upon, investigation it was found out that the medicine the
patient took that night supposedly for her anxiety was HYDRALAZINE. The hospital
coordinated with the patient’s physician, and the latter was told about the incident that took
place.

The physician said that he prescribed HYDROXYZINE and not HYDRALAZINE. After, finding out
that there was a mix-up in her medications, Karen filed for legal charges to the only person
she can blame for the incident - the dispensing pharmacist. In an interview with the dispensing
pharmacist, she said she was sure that she dispensed Hydroxyzine, however she did not let
any of her co-workers double check the medication, because it was a busy hour in the
pharmacy.
