In light of the recent data quality check conducted on Kapai RHU and its corresponding staff. I
heartily apologize for failing to meet the standards required of my employment. Specifically, My failure
in maintain and updating documents in spite of my regular attendance. I understand that my work is
mostly client-based, it is with utmost importance that I ensure the accuracy and veracity of
documentation at all costs. While the negligence on my part is inexcusable, I will duly rectify myself for
the lapses of my performance.
