Dear Ma’am,
Greetings of Peace!
We are presently conducting a study entitled “Lived Experiences of Pregnant Women on the
Outbreak of COVID-19 Pandemic”. One of the requirements in order to complete our research
paper is to conduct an interview among our identified respondents. It aims to gather relevant data
as our bases in providing the results and discussion of our study. Thus, it will be done through
face to face interaction considering the safety protocols.
In line with this, we would like to seek approval from your good office to allow us to conduct the
interview to the chosen respondents in Barangay Dualing, Aleosan,North Cotabato. Rest assured
that any data or information gathered from participants will be handled with utmost
confidentiality so us to violate any ethical standards that would compromise the privacy rights of
our respondents. Moreover, we assure you that minimum health standards will be observed on
the pursuit of the study at all times
We are hoping for your support and approval in this matter. Thank you very much and may God
bless you always.
Respectfully yours,
