There are 6 criteria in modified altered scoring

 ACTIVITY (movement)
SCORE = give 2 points if the patient was able to move all 4 limbs(upper and lower limb)
=give 1 point if the patient was able to move any two limbs only
= give 0 point if the patients is unable to move any limb

 RESPIRATION (BREATHING)
SCORE = give 2 points if the patient was able to breathe deeply and cough freely
= give 1 point if the patient experiencing dyspnea
=give 0 point if the patient is unable to breath

 Circulation (blood pressure)


SCORE = give 2 points if the patient blood pressure +/- 20 mmHg of normal
= give 1 point if the patient blood pressure +/- 20-50 mmHg of normal
= give 0 point if the patient blood pressure +/- more than 50 mmHg of normal

 CONCIOUSNESS (responsiveness)
SCORE= give 2 points if the patient is fully awake
=give 1 point if the patient is arousable on calling
=give 0 point if the patient is not responsive

 OXYGEN SATURATION (as determined by pulse oximeter)


SCORE = give 2 points if the patient was able to maintain SpO2 greater than 92% on room air
= give 1 point if the patient needs supplementary O2 to maintain SpO2 greater than 90%
=give 0 point if the patient SpO2 lesser than 90% despite supplementary O2

 PAIN
SCORE = give 2 points if the patient is minimal to no pain or controllable with oral analgesics
=give 1 point if the patient encounter presence of pain despite analgesic
 SURGICAL BLEEDING
SCORE = give 2 points if minimal/does not require dressing change
=give 1 point if up to two dressing changes required
=give 0 point if more than three dressing changes required

 NAUSEA AND VOMITING


SCORE = give 2points if none to minimal
= give 1 point if moderate
=give 0 point if severe
