To receive your UK Herbalife earnings you must complete this Electron Fund Transfer (EFT) form and return it to Herbalife (UK)
Limited. For assistance in completing this form, please refer to the corresponding Guidelines.
 All changes will be made and any earnings paid upon completion of this form and its processing by Herbalife UK Limited.
 Please allow 10 business days for Herbalife to process this form.

By completing, signing and returning this form to Herbalife, I authorise and consent to Herbalife collecting, using and disclosing my personal information as
required for the purpose of depositing my net earnings and any other sums (“Deposit”) due to me from Herbalife in the Account identified above.
This authorisation and consent shall remain effective until revoked by me in writing. I understand and agree that Herbalife shall take: (i) reasonable steps to
ensure that information that is collected from me is accurate and as up to date as possible; and (ii) appropriate steps to keep my information secure while it
is in Herbalife’s possession. I also consent to Herbalife sending my personal information outside the EEA to Herbalife in the US for the purpose of
depositing the Deposit due to me from Herbalife in the Account identified above. I understand and agree that Herbalife shall have no liability for the failure of
any deposit to reach my account in a timely and accurate manner, although, in the event of any error, Herbalife will fully cooperate to rectify such error.
Since no data transmission can be guaranteed 100% secure, I acknowledge and agree that Herbalife shall not be responsible for any harm that results from
any breach of confidentiality arising out of any communication with, or transmission to, my financial institution in connection with any Deposit.
