Quick steps to complete Online application!
• Go to https://www.myherbalife.com/en-in/ to access the online Application and click on “Apply Online” located within
the “Join Herbalife!” box.
• Create Herbalife account & fill up the required details online
• Acknowledge Terms & Conditions
• Deselect the box if you do not have a paper application & submit to generate new ID number

• Scan copies of Proof of Address, Proof of Identity, FSSAI Document

• What’s new?
For Preferred Customer - No need to print a copy of application & Self attest your Proof of Identity. Alternatively, we request
you to submit copy of Proof of Identity & signed Self Declaration letter.
For Associate - No need to print a copy of application & Self attest your Proof of Address, Proof of Identity, FSSAI
Registration/ License. Alternatively, we request you to submit copies of Proof of Address, Proof of Identity, FSSAI
Document & signed Self Declaration letter.
For Associate to Preferred Customer Conversion - No need to print a copy of application agreement & Self attest your
Proof of Identity. Alternatively, we request you to submit copy of Proof of Identity & signed Self Declaration letter.
For Preferred Customer to Associate Conversion - No need to print a copy of application agreement & Self attest your
Proof of Address, Proof of Identity, FSSAI Registration/ License. Alternatively, we request you to submit copies of Proof of
Address / Proof of Identity, FSSAI Document & signed Self Declaration letter.

• Upload the scanned above documents & signed Self Declaration letter through intelligent channel on
https://www.myherbalife.com/en-in/ or use the I Channel link sent to your registered email address.
• Once all above documents are uploaded, please respond and reply to email received on your registered email address to
confirm /acknowledge the submission of documents.
• Click on HYPERLINK http://www.myherbalife.com/en-IN/Training/Courses/ds for new Associate to complete your Mandatory
Orientation Training (MOT)

acknowledgement form which has application reference number
