Guidance Notes:
Students should state the procedure to be carried out.Students should explain how the
clinincal area was made ready for this particular procedure, how they assisted both the
clinician and the patient during the procedure, and to be able to describe their role during
the procedure.Students should describe the patient on which the treatment was carried out.
PATIENT INFORMATION: Sex: Male Age: 24 year old - NHS patient; Medical/Social
History form up to date and signed; Health: Good. Patient concert: Gained, plan signed.
- All surfaces in surgery are wiped with non-alcohol free disinfectant wipes, dentist chair
water lines flushed for a minimum of 30 seconds (between patients, 2 min at start/end of the
day)
- Surgery zoned into clean and dirty areas with a decontamination room adjoined.
- Dentist and myself follow the 13 steps hand washed done by NHS, placed new PPE gloves,
mask and my safety glasses.
- I select on the computer patients name, and leave open patient's records.
- Examination tray:

Polishing disposable brush
Set of scalers (sickle, briault, jacquette)
3 in 1 metal air/water syringe
Ultrasonic scaler machine

PROCEDURE:
- I bring patient into the surgery and provide a bib and safety glasses, always make sure and
ask if patient is comfortable to start the procedure.
- Whilst dentist start scaling around the gums and polishing, I keep all mouth area dry and
clean from bites of plaque aspirating with fast suction tube and ejector saliva.
- Explanation given to patient about ‘dental cleaning’ and along with tooth brushing,
flossing, mouthwashes and healthy eating, good diet, give up of bad habits, can help to
reduce the risk of gum disease and tooth decay.

A scale and polish is a very common dental treatment which is carried out as a form of oral
preventative medicine. In other words, it is designed to form part of a dental hygiene plan
with the aim of keeping your teeth clean and healthy.
Patients go home with other attitude, more conscious about the risks, paying attention on
sugar levels, and time spent brushing their teeth every day.

I should be more quicker. Need to improve my computer skills to help dentist to record
patient's record.
- I do have good computer skills, but because the software is a type of software's we dont use
at home, is just for work (dentists software) was not familiar for me. Now I am getting used
to.

most ways it's fine and I would complete this pers - you have not put patient type so I have to
send it back
**In general speed is not something I like to see as a weakness or an aim - it can make us
forget things, drop things and also cut corners - I would like you to become more efficient
and successful
