Dear Ms. Sibug,

This is to formally inform you that you will be reporting onsite starting September 16, 2021,
Thursday. This due to the following reasons:

a. Poor / unreliable connection was reported by you during critical hours


b. Overbreak during lunch
c. Failure to report to immediate supervisor or report to wah gc of the challenges for not
reporting on time or logging in back ontime
d. Failure to report onsite for team update every Monday and Friday
e. Failure to follow action plan as agreed before wah was implemented
f. Failure to answer the call when a supervisor / manager is calling you
g. KPI on a monthly basis.

You were reminded multiple times to correct these behavior as this affect your productivity. Also we
have given you ample time to adhere to your commitment but you have ignore our guidance and
warning. Your WAH login will be temporarily terminated for the meantime starting September 16,
2021.

Insubordination will be filed if this notice will be ignored as stated in the Sykes Handbook.
Over break and under time are other forms of tardiness and neglect of duty which adversely affects
business operations. All employees are expected to know their given schedules and to be mindful
about over extending breaks as provided or leaving work before the end of their shifts.

Overbreak or undertime for three (3) times or an accumulation of 10 minutes, whichever comes first
in one calendar month shall also be construed as a single violation.

Further, should an employee be found to have committed over break or under time again within the
same calendar month, these shall be treated as separate instances of overbreak or undertime and
shall be dealt with in accordance with the tardiness penalty table as outlined below.

Finally, once a person is meted three (3) sanctions for overbreak or undertime within a six (6) month
period, any succeeding minute or instance of overbreak or undertime shall be a violation and shall
follow the above penalty grid.

Any employee found to be logged into the time and attendance system but not found at his or her
post shall be subject to violations under the Sykes Code of Conduct.

DISCIPLINARY ACTION CLEANSING PERIOD:


Tardiness, Overbreak or Undertime, cleansing period for all penalties shall be six (6) months.

The cleansing period shall be counted from the date of issuance of the Show Cause/ Corrective Action
Memo for the latest offense.

In view thereof, please explain in writing within twenty-four (24) hours upon receipt of this memo
why you should not be held administratively liable for violating the Company’s Attendance Policy,
Section 5. Absences and Tardiness, Subsection J. Excessive Tardiness. Tardiness for three (3)
instances or an accumulated tardiness of fifteen (15) minutes, whichever comes first, within any
calendar month shall constitute excessive tardiness and shall be considered as a violation. Moreover,
once a person is meted three (3) sanctions for tardiness within a six (6) month period, any succeeding
minute or instance of tardiness shall be a violation and shall follow the applicable penalty grid.

The aforesaid violation/s if proven true is punishable with the penalty of Written Counseling to
Termination depending upon the gravity of the offense/s and of previous related offense/s.

Your failure to submit said written explanation within the period provided herein shall constitute a
waiver on your part and shall constrain Management to decide on the administrative case based on
the records and evidences at hand.
