5. Application for Order of Survey
6. Environmental Compliance Certificate (ECC)
7. Environmental Protection and Enhancement Program (EPEP)
8. Final Mine Rehabilitation and/or Decommissioning Plan (FMRDP)
9. Social Development and Management Program (SDMP)
10. Safety and Health Program (SHP)
11. Final Exploration Report/Ore Resource Report (FER/ORR)
12. Project approval/ endorsement by the Sanggunian concerned
