Subject: SCHOOL DISTRIBUTION AND RETRIEVAL OF THE NEW-NORMAL HEALTH
ASSESSMENT FORM FOR LEARNERS
1, Health Assessment plays a vital role in evaluating and determining a learner's health
status. This will identify the specific needs of the person and how those needs will
be addressed.
I, In line with this and in lieu of physical examination, each school is advised to adopt
the New Normal Health Assessment Form for Learners. This will help detect the
early signs of certain health conditions and address the accordingly. Likewise, this
will aid in the profiling of the learners and serve as basis for future stakeholders’
support,
Ill. — With this, the New Normal Health Assessment Forms shall be distributed by the
Division Health and Nutrition Personnel to the schools of the different districts.
IV, Attached herein is a copy of the New Normal Health Assessment Form for Learners.
