Introduction

Patients’ and their families’ independence has been a topic of discussion since many years and

many studies have been conducted to reveal its importance. The independence in this context is

basically an ability to achieve, to make the decisions and to initiate the actions. The health care

professionals are not only responsible for providing the medical care but they must enable a

balance between dependence and the independence to improve the patients’ and their families’

ability maximize the outcome of care and have autonomy. In this regard the underlying report is

based on 3 different scenarios that are to be addressed on behalf of a health care professional.

The aim of the report is to propose the effective strategies and actions which can enable the

patients and their families to achieve the maximum independence and analysing the factors

which could affect the patients’ physical, psychological and social well being.

Scenario 1: An elderly lady is living at home, but is partially sighted

1. Effective strategies and actions to achieve maximum independence

As a health care provider to a lady who is partially sighted, the very initial strategy will be

promotion of self-management. This strategy is proposed because it is a structured way that will

give the patient all the knowledge and skills she need to handle her medical condition that is the

the inability to properly sight. Another very important action which will increase the

independence of not only the patient but all her family is management of the emotional and

social issues which are surrounding the long term illness. For managing the emotional and social

issue the best strategic action is the empowerment of patient as well as the family so that they

could control the care and recognize the importance of the ongoing professional support that has
been and will be delivered to them. This strategy of empowerment is consistent with the new

policy of National Health Service in UK which has proposed a new care model which is based

on empowering patients and the communities (NHS, 2015).

2. Two factors that could affect their Physical wellbeing

A main factor which can influence the physical health of the lady with partial sight is the self-

efficacy. It is basically a belief that a patient tends to have in him or herself that he or she can

accomplish a given task. In this regard, the focus will be placed on increasing her self-efficacy so

that he could have more perception of control, reduced emotional stress and high activity level.

The send factor is problem solving which is directly related to her disease. As she cannot

properly sight, active learning will be used to support her problem solving skills (NHS, 2015).

3. Two factors that could affect their Psychological wellbeing

For improving the psychological health the main factor is the coaching. It will promote self-

management skill sand the patient and her family’s learning to handle the medical problem. And

the second factor which can improve the psychological health is ability to share. In this regard,

High Court of UK has ordered to provide a “safe space” to patients and HSSIB has a new feature

which is the new approach to investigate the issues faced by families and patients themselves. In

this regard, safe space will be provided so that patients and families could share their concerns

and benefit from the medical care delivered (Gov.UK, 2017).

4. Two factors that could affect their Social wellbeing

To improve the social wellbeing, motivational interviewing and factor of partnership must be

used. In England motivational interviewing and emphasized partnerships are valued as they are
claimed to be responsible for improving long-term social health of patients and these are made

apart of NHS heath check guide (Royal Pharmaceutical Society , 2018).

Scenario 2: A lady has just had a C-section and is now back on the ward

1.Effective strategies and actions to achieve maximum independence

The first strategic action will be using the SMART goals for example motivating the women to

walk for certain minutes post the c-section to feel her empowered again. The second action will

be addressing the barriers she is facing such as cultural issues which are making it difficult for

her to carry out the goals. These strategic actions are consistent with the guidelines given by

National institute for the health and care excellence in 2011 (NICE, 2018).

Advocacy and the support for the effective decision making can both improve the physical

health of the women after c section. Problem solving is also an important aspect of self-

management and the advocacy can deliver improved care (Gov.UK, 2017).

3.Two factors that could affect their Psychological wellbeing

NICE‘s guidelines also includes a clause of providing Kind, friendly and professional services.

So the two factors that will improve psychological health will be kindness, and friendliness

which will promote the communication and will eliminate the barriers (Young, 2018).

4.Two factors that could affect their Social wellbeing


For improving the social wellbeing of the woman patient-cantered care will be delivered instead

of service-led are. And the nurse-patient relationship will be improved so that healthy behaviour

could be promoted.

Scenario 3: A male soldier is recovering following an amputation

1.Effective strategies and actions to achieve maximum independence

When promoting independence of the patients which is a soldier recovering from an amputation

and his family, the best strategy would be empowerment. This strategy is suitable for the patient

himself as well as his family who must have more control and encouragement to recover from

physical, emotional and psychological challenges. A relevant action will be use of self-

management programme which will improve patient-professional partnership and will improve

quality of care. These strategies and actions are proposed in compliance with the priority NHS

treatment for the veterans (NHS , 2018).

2.Two factors that could affect their Physical wellbeing

According to the legislations of Defence Medical services the soldier’s physical wellbeing will

be improved by two factors including exercise therapy and rehabilitation services (Gov.UK,

2018).

3.Two factors that could affect their Psychological wellbeing

Two factors for improving the psychological health of oldie will be education and support to

meet the self-care needs. A supportive environment will be provided so that heath care outcomes

could be optimized.
4.Two factors that could affect their Social wellbeing

For improving the social health and wellbeing compliance with the guidelines of Headley Court

Defence Medical Rehabilitation enter will be ensured. The two factors will be coaching to clarify

level of responsibility to be taken off by patients and his family and the improvement of self-

esteem through discussions (NHS , 2018).

Conclusion

Three different scenarios have been analysed and alternative strategies, actions and factors have

been discussed. Effective strategies and actions proposed includes, self-management,

empowerment, use of SMART goals, overcoming barriers, patient-professional partnership etc

all of which enables the patients and their families to attain maximum independence. Various

factors that can positively influence the patients’ physical, psychological and social well being

have also been discussed and it has been concluded that quality of care is dependent upon the

effectiveness and efficiency of selecting and applying relevant strategies an actions by the

healthcare professionals and those initiatives must be consistent with the relevant legislations and

must be consistent with the nature of disease.
